Letter of Completion Request Procedure


CPR SQUARE
is a training site under The CPR Lady Training Center. Please follow the procedure below to receive the letter of completion.

The Letter of Completion is on Training Center letterhead signed by the Training Center Coordinator for a candidate applying to a California State Board. Letter states student completed the AHA course and holds a valid AHA card. There are two options to request the letter.

Option #1 – By Mail

All items below are REQUIRED to process your request:

  • Enclose a note that includes as much of the following information as possible. The more information provided, the easier it is to find your course entry and compose the Letter of Completion. Letter will be on Training Center letterhead and signed by the Training Center Coordinator stating student completed the AHA course and holds valid AHA card.

    • Name on card

    • Class type (BLS, Heartsaver, etc.)

    • Date of class

    • Instructor name

    • Your contact information (phone # / email) if additional information is required

  • Enclose a self-addressed stamped envelope to mail your Letter of Completion back to you. Use a standard letter #10 envelope. The address you write on this envelope is where the letter will be mailed. Postage is REQUIRED, meeting USPS First Class Postage rate.

  • Letter of Completion fee – $15.00 in the form of cash or check (no credit cards accepted). Make check payable to The CPR Lady Inc.

Option #2 – Phone/Pick Up

When the student has an urgent need for a Letter of Completion, contact The CPR Lady Inc. Training Center by phone. Letter will be on Training Center letterhead and signed by Training Center Coordinator stating student completed the AHA course and holds valid AHA card.
